New Hire Paperwork

Please be sure to read all the information on this page to avoid delays in starting work due to incomplete paperwork.

By law, all (part-time, full-time, and seasonal) employees must complete the documents below:

• I-9 (PDF)
• W4 (PDF)
• CORI (PDF)
• Social Security Notice

In addition to the required Paperwork above, employees also need to print and complete the following appropriate packets:

• Part-Time Hire Paperwork (PDF)
• Full-Time Hire Paperwork (PDF)
• Poll Worker Paperwork (PDF) - Please Note: Poll Workers Must also print and complete the W4 Form (PDF). Please print and Complete all paperwork before returning to the Winthrop Town Clerk's office.  Incomplete paperwork will delay your ability to work.  Please pay special attention to the acceptable forms on the ID page of the I-9.  Not having the correct forms of ID will also delay your ability to work.
• Parks and Recreation Paperwork (PDF) - Please Note: Parks and Recreation employees must also print and complete the W4 Form (PDF). Please print and Complete all paperwork before returning to the Park and Recreation Department.  Incomplete paperwork will delay your ability to work.  Please pay special attention to the Acceptable Forms of ID Page of the I-9 (PDF).  Not having the correct forms of ID will also delay your ability to work. Employees 18 and over must complete the CORI (PDF).
