The DPW Permit Application for Street Excavation, Underground Utility Repair, Street or Sidewalk Obstruction, or Driveway Installation or Repair may be printed, completed, and brought into the DPW at 100 Kennedy Drive. Please complete BOTH pages of the application. If you are unable to print and complete the application, you may request one at the DPW.
